Please send filled out forms to Trinitymassagesllc@yahoo.com 24 hours prior to your appointment. This helps to ensure you get the most out of your session.

I must have client intake, cancellation and booking policy, as well as liability forms filled out before you can be seen. If you want cupping and hot stone those are needed as well.

if you're coming in for a prenatal or bringing your child for massage then please also fill out those forms.
